1. Fill out the patient history and health questionnaire in  Reveliadx
2. Log in to our Electronic Medical Record for communication and to retrieve your lab requisition.
3. Get your blood drawn at Quest, Labcorp or another lab location near you. 
4. Contact us with your scheduled lab draw, so we can schedule your appointment
